Insurance
We participate with many health maintenance organizations and preferred provider organizations. Please check with our front office personnel or your health plan to determine if we are an approved provider for your insurance carrier. Such plans will be billed by our office for services rendered according to terms set forth in our contract with that particular carrier. You will be required to pay any co-payment, co-insurance or deductible at the time of the office visit.
If you have other private insurance, payment is required at the time of the visit. You can make payment by cash, check or credit card. A receipt will be provided with the necessary information that will allow you to file your insurance.
